Because the office of Dr. Jeff Matilsky knows how important your time is, we do our best to stay on schedule to minimize your wait. Prior to your first appointment, you will be asked to arrive fifteen minutes early, so we may finish gathering your personal information. You may download our forms from this site to help minimize the time.

We would appreciate if you would bring along with your forms already filled out, the following information:

• Recent x-rays within the last year, if available.
• Make sure you have a list of all medications you may be taking. If you have the need to premedicate due to your medical history take an antibiotic prior to a dental appointment.
• Check with your physician if you have been diagnosed in the past with any of the following: a heart murmur, artificial heart valve or joint replacement.

A parent or guardian must accompany patients under 18 for this visit.
